For medical billing purposes, the Date of Service is:
the date the patient (or patient representative-guardian or practitioner) receives the completed product.
the date the orthoses are delivered to the address on the SHIP TO part of the order form.

Important Note
The date of service is not the date the patient is cast, nor the date the order is placed, nor the date of the fitting appointment.
